Thank you for completing your consent form, we will review it momentarily and follow up if we have any additional questions.
A team member will send you a text message within 30-45 minutes of Dr. Chan’s arrival; please remember that your appointment time is a time window to allow for traffic variables. We will do our best to update you if traffic conditions change, but please be aware that we may not be able to give you any new information until she is finishing the appointment immediately before yours.

***Additional clay prints and ink prints (only available with aftercare services) MUST be requested before the end of your visit or they cannot be guaranteed***
How will ashes be returned to me?
Ashes are shipped at no additional fee with a tracking number - a signature IS required and any packages returned to us will be subject to a redelivery fee
Complimentary hand delivery is offered for local clients only and is subject to our scheduling limitations; these deliveries are typically offered Friday-Sunday. We will reach out via text with the time window we have available once your pet’s ashes are ready to come home. Unfortunately we cannot accommodate requests outside of the day/time provided but we can reschedule to an upcoming delivery date.
For clients needing a specific time/date or outside of our local area, hand delivery is available for an additional fee ($100-175); distance limitations will apply
In person pick up is only available from the crematorium in Lake Elsinore as Dr. Chan is mobile only and does not have an office location available to clients - this decision must be finalized before the end of your appointment as any changes will delay returning your pet’s ashes.
